This position will also sees hospice patients.

What You’ll Do

As a Home Health Aide, you’ll be a vital part of our care team by:

• Bringing each patient’s Plan of Care to life
• Assisting patients and families with personal care and activities of daily living
• Providing home health or hospice visits throughout a designated service area
• Completing timely and accurate documentation to support excellent patient care
