Duties

• Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
• Monitor and observe patient health status, including vital signs and overall well-being, while accurately documenting medical information in Electronic Health Record (EHR) systems
• Administer medications following medical instructions and ensure proper medication documentation in compliance with home healthcare regulations and HIPAA privacy standards
• Provide specialized care for patients with dementia, Alzheimer's disease, or other cognitive impairments, including memory care strategies and behavior management
• Support patients with complex needs such as catheter care, hospice care, end-of-life assistance, and managing medical devices or equipment
• Prepare nutritious meals tailored to dietary restrictions and assist with feeding as needed; perform light housekeeping tasks like cleaning and laundry to maintain a safe environment
• Use medical terminology confidently to communicate effectively with healthcare teams and document patient progress accurately

Experience

• Prior experience working with geriatric patients or in senior care settings is highly preferred
• Demonstrated assisted living experience or home healthcare agency background is advantageous
