We are seeking a professional, compassionate, and dependable Certified Nursing Assistant (CNA) to join our team serving Chandler & Tempe / Awtukee.

Role Summary

The CNA provides essential personal care and supportive services to patients in their homes. The ideal candidate is compassionate, dependable, professional, and committed to providing safe, respectful, and high-quality patient care.

Key Responsibilities

• Assist patients with personal care, including bathing, grooming, dressing, toileting, and light housekeeping.
• Measure and record vital signs as directed by the supervising clinician.
• Assist with mobility, safe ambulation, transfers, and prescribed exercises according to the care plan.
• Observe patients for changes in condition and promptly report concerns to the assigned nurse.
• Provide companionship and support while promoting patient dignity and independence.
• Maintain accurate and timely documentation of all care provided.
• Follow established care plans, agency policies, and patient safety procedures.
• Maintain confidentiality and comply with HIPAA/PHI, infection control, and safety standards.

Required Qualifications

• Active Certified Nursing Assistant (CNA) certification/license
• Current CPR certification
• Valid Arizona Fingerprint Clearance Card
• COVID-19 vaccination, when required by applicable CMS regulations
• Reliable transportation for in-home patient visits
• Ability to follow care plans and accurately document services provided
• Ability to work independently and professionally in a field-based environment

Preferred Qualifications

• Minimum 1 year of hands-on CNA experience
• Home health experience strongly preferred
• Experience working with geriatric or medically complex patients is a plus

Pay rate

$25 – $28 per visit, depending on experience and qualifications.
