Job Description

Sierra Nevada Home Health is hiring a compassionate and dependable Certified Nursing Assistant (CNA) to join our growing home health team!

We are looking for a motivated CNA who enjoys providing one-on-one patient care in the comfort of patients' homes. This is a per diem position offering flexible scheduling and the opportunity to make a meaningful difference in the lives of patients throughout Carson City and surrounding communities.

Our team is dedicated to providing high-quality, patient-centered care while promoting independence, dignity, and safety.

Responsibilities

• Provide personal care and assistance with activities of daily living (ADLs).
• Assist patients with bathing, grooming, dressing, and personal hygiene.
• Assist with ambulation, transfers, and mobility as appropriate.
• Monitor and report changes in patient condition to the supervising nurse.
• Provide skin care and assist with infection prevention.
• Maintain patient safety and comfort during home visits.
• Document patient care accurately and in a timely manner.
• Communicate effectively with patients, families, and the clinical team.
• Follow established care plans and agency policies.
• Promote patient independence and dignity.

Qualifications

• Current Nevada CNA certification in good standing.
• Current CPR certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to travel to patients' homes throughout the assigned service area.
• Strong communication and time-management skills.
• Ability to work independently in a home health setting.
• Previous home health or patient care experience preferred.
• Compassionate, dependable, and professional attitude.
