Certified Nursing Assistant (CNA)

Home Health CNA

We are seeking experienced and compassionate Home Health CNAs to be a part of our growing care team. We pride ourselves on providing excellent service to our patients and maintaining active communication and involvement with the loved ones who assist in their care. In order to consistently provide that same level of care, we want team members who have strong relationship building skills and a compassionate attitude that will allow our patients and their families to feel comfortable and confident in the support they will be receiving. RESPONSIBILITIES

• Collaborate with physicians and other professionals to plan and implement care
• Make frequent visits to the patient’s home to assist with daily care needs and observe their condition
• Provide support and comfort according to each patient’s individual needs
• Work with caregivers to plan and execute excellent patient care
• Assist with personal care, including bathing, grooming, dressing, toileting, and mobility
• Help with medication reminders as permitted by state regulations and agency policy
• Monitor and record vital signs
• Report changes in a patient’s condition to the supervising nurse or physician when deemed necessary
• Assist patients and their families with paperwork
• Maintain accurate records
• Educate and support family members and caregivers in ongoing patient care

REQUIREMENTS

• Reliable transportation
• Finale Health EMR experience preferred
• At least 1 year of home health experience preferred
• Valid CNA license
• Valid CPR certification
• Strong written and verbal communication skills
• Strong relationship building skills
