Certified Nursing Assistant (CNA) – Hospice

APEX Hospice & Palliative Care is seeking compassionate, dependable Certified Nursing Assistants (CNAs) to join our growing hospice team. We are currently hiring for the North Side of Chicago, Lemont, and McHenry/Lake County service areas.

As a Hospice CNA, you will provide personal care and emotional support to patients with life-limiting illnesses in their homes and long-term care facilities while working closely with an experienced interdisciplinary team dedicated to exceptional end-of-life care.

Responsibilities

· Provide personal care, including bathing, grooming, dressing, toileting, and oral care.

· Assist with ambulation, transfers, and repositioning.

· Observe and report changes in patient condition to the RN Case Manager.

· Document care accurately and timely.

· Support patients and families with dignity, compassion, and respect.

· Maintain infection prevention and safety standards.

Qualifications

· Current Illinois Certified Nursing Assistant (CNA) certification.

· Current driver's license, reliable transportation, and auto insurance.

· Ability to travel throughout the assigned territory.

· Excellent communication and interpersonal skills.

· Hospice or home health experience preferred but not required.
