Responsibilities

• Deliver direct patient care by assisting with activities of daily living such as bathing, dressing, feeding, and mobility support using equipment like Hoyer lifts.
• Monitor and record vital signs accurately, including blood pressure, pulse, respiration, and temperature, ensuring timely documentation in electronic health record (EHR) systems like Matrix
• Assist with specialized care needs such as dementia, Alzheimer's, memory care, behavioral health issues, and working with individuals with developmental disabilities or disabilities.
• Maintain infection control standards through proper use of PPE
• Document all patient interactions thoroughly in medical records and ensure compliance with standards and nursing home protocols.

Qualifications

• Current CNA certification with valid state licensure required.
• Experience working in diverse healthcare settings such as hospitals (ICU, emergency medicine), nursing homes, long-term care facilities, assisted living, or home & community care is highly preferred.
• Familiarity with EMR/EHR systems including Matrix or similar platforms for accurate medical documentation.
• Knowledge of physiology, anatomy, medical terminology, and vital signs measurement techniques essential for providing effective patient service.
