Your energy and dedication will help foster a nurturing environment that promotes health, dignity, and independence.

Responsibilities

  • Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and other assistive devices.
  • Monitor and record vital signs including blood pressure, pulse, respiration rate, and temperature accurately within electronic health record (EHR) systems such as Kinnser (WellSky).
  • Administer medications and perform basic medical procedures following strict aseptic techniques and HIPAA compliance to ensure patient safety and confidentiality.
  • Support patients with complex needs including Alzheimer's care, dementia care, memory care, behavioral health challenges, and post-acute recovery processes.
  • Collaborate with interdisciplinary teams in settings like medical-surgical units, ICU, emergency departments, hospice care, or outpatient clinics to deliver comprehensive patient-centered care.
  • Document all patient interactions thoroughly in medical records using EMR systems like eClinicalWorks or Meditech to ensure accurate medical documentation.
  • Assist with specialized procedures such as catheter care, phlebotomy (blood draws), and infection control practices in accordance with hospital protocols and Level I/II trauma center standards.

Skills

  • Proven experience working in diverse healthcare environments including hospitals (ICU, PICU), nursing homes, assisted living facilities, or home health settings.
  • Strong knowledge of medical terminology, anatomy, physiology, and patient care techniques across pediatric to geriatric populations.
  • Familiarity with EMR/EHR systems such as Epic, Cerner, or eClinicalWorks for documentation and patient record management.
  • Ability to perform medication administration safely and accurately following established protocols.
  • Experience caring for individuals with developmental disabilities or behavioral health needs; knowledge of dementia and Alzheimer's disease is highly valued.
  • Competency in vital signs measurement, aseptic technique, catheter care, and infection prevention practices.
  • Excellent communication skills to effectively collaborate with healthcare teams and provide compassionate patient service across inpatient and outpatient settings. Join us to be part of a passionate team committed to delivering exceptional care that transforms lives! We value your expertise in senior care, acute care environments like trauma centers or emergency medicine units—and your dedication to supporting patients through every stage of their health journey.
