Position Overview

We are seeking a dedicated and compassionate Certified Nursing Assistant (CNA) to join our hospice and home health team. This role provides direct patient care under the supervision of a registered nurse, helping patients maintain comfort, dignity, and quality of life in their own homes or care settings.

Responsibilities

• Provide personal care to patients (bathing, dressing, grooming, toileting, mobility assistance)
• Assist with basic health needs as directed by the nurse (vital signs, simple wound care support, range of motion exercises)
• Offer emotional support and companionship to patients and families
• Maintain accurate and timely documentation of care provided
• Communicate changes in patient condition promptly to supervising nurse
• Travel to patient homes or facilities within the Salt Lake City area

Qualifications

• Current Utah CNA certification (required)
• Current driver’s license, reliable transportation, and proof of auto insurance
• Previous hospice or home health experience preferred, but not required
• Compassionate, dependable, and able to work independently
• Strong communication and interpersonal skills
