### Job Responsibilities

As a CNA, you will provide personal care and support to home health and hospice patients under the direction of a Registered Nurse (RN). Duties include:* Assisting patients with personal hygiene and daily living activities according to their care plans * Providing approved CNA and home health or hospice aide services within your training and certification * Monitoring patients and reporting changes in their condition to the care team * Maintaining accurate and timely patient care documentation * Safely using medical equipment within your training and abilities * Following patient care instructions, infection control procedures, and safety guidelines * Communicating professionally with patients, families, and healthcare providers * Promoting patient comfort, independence, and dignity

### Required Certifications & Licenses

• Current Utah CNA certification
• Valid CPR certification
• Valid Utah driver's license
• Current automobile insurance
