Duties

* Assists clients with activities of daily living: healthy meal preparation, personal care, light housekeeping, errands, companionship, transportation, etc.

* Create Happier Aging experiences and client focused care in home and community settings

* Transfer clients, assisted, unassisted, or with equipment such as hoyer lift

* Provide personalized care and adapt to client care needs (stroke, memory care, incontinence, immobility, etc

* Document care provided and communicate any changes in client condition to supervisors promptly
