### Job Summary: Direct Support Professional (Caregiver)

The successful candidate will assist with daily routines, encourage independence, and provide support while engaging in activities such as fitness, recreation, and community outings.

### Shift Details:

• Schedule: Monday, Wednesday, and Friday
• Hours: Approximately 10:30 AM – 3:30 PM
• Flexibility: Some schedule flexibility may be available and will be discussed during the interview process.

### Core Responsibilities:

• Provide one-on-one support and companionship while encouraging independence.
• Assist with community activities, errands, appointments, and recreational outings.
• Provide transportation as needed.
• Encourage participation in healthy activities, hobbies, and interests.
• Assist with daily routines and provide support based on individual needs.
• Complete required documentation accurately and timely.
• Maintain professionalism, confidentiality, and respect at all times.

### Requirements:

• Must be at least 18 years of age.
• Must have a valid driver’s license.
• Must have reliable transportation.
• Must provide valid auto insurance and vehicle documentation.
• Must pass all required background checks and screenings.
• Must have a dependable, patient, and positive attitude.
• Must be comfortable providing community-based support and engaging in recreational activities.
