DSP/PCA/Caregiver

POSITION SUMMARY

The Direct Support Professional provides person-centered support services to individuals with disabilities in home and community-based settings. Services are provided in accordance with Minnesota Statute Chapter 245D and each service recipient's Individual Service Plan (ISP). The DSP works to support independence, community integration, health, and quality of life.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Provide personal care and assistance with activities of daily living (ADLs) as outlined in the ISP.
• Implement positive support strategies and behavior support plans as specified in the ISP.
• Assist service recipients with community integration, transportation, and participation in activities.
• Document all services provided, incidents, and significant events accurately and timely per agency standards.
• Report any suspected maltreatment, abuse, neglect, or exploitation immediately per Minn. Stat. 245A.65 and 626.557.
• Respond to medical emergencies and contact emergency services as needed.
• Maintain confidentiality per HIPAA and Minnesota Data Practices Act (Minn. Stat. 13).
• Attend all required training, supervision meetings, and staff meetings.
• Follow all agency policies, procedures, and 245D regulations.

MINIMUM QUALIFICATIONS

• High school diploma or GED equivalent.
• Must be at least 18 years of age.
• Cleared DHS Background Study (Minn. Stat. 245C) prior to working with service recipients.
• Ability to read, write, and communicate effectively in English.
• CPR/First Aid certification required within 90 days of hire.

PREFERRED QUALIFICATIONS

• Prior experience supporting individuals with intellectual/developmental disabilities or mental health conditions.
• CNA certification or related healthcare credential.
• Knowledge of 245D regulations and person-centered planning.
