Job Summary

The Home Health Aide (HHA) provides direct patient care under the supervision of the RN Case Manager, contributing to the delivery of safe, high-quality home healthcare services. The HHA follows an individualized care plan and supports activities of daily living to ensure patient comfort and well-being.

This position is available for pediatric clients in school and home setting

Key Responsibilities

· Assist patients with personal care including bathing, grooming, toileting, oral hygiene, nail care, and dressing.

· Support mobility through ambulation assistance, transfers, and use of mechanical lift devices.

· Perform light housekeeping, meal preparation, and bed making.

· Monitor and report vital signs (temperature, pulse, respiration)..

· Provide emotional support and companionship.

· Observe and report changes in the patient's condition to the RN Case Manager.

· Document care and interactions per agency policies.

· Maintain a clean, safe, and healthy environment for patients.

Qualifications/Requirements

· Minimum 18 years of age.

· Active HHA certification or Certified Nurse Assistant license; or willingness to complete HHA certification within 90 days of hire.

· Reliable transportation and communication methods.

· Valid driver's license and vehicle insurance (if transporting clients).

· Ability to pass criminal background and FBI biometric checks.

· Physical capability to perform lifting (up to 75 pounds), extensive bending, and standing for up to 6 hours daily....
