Job Overview

We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) or Home Health Aide (HHA) to join our dynamic healthcare team. Responsibilities

• Deliver direct patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts.
• Monitor and observe patients’ conditions, including vital signs, and report changes promptly to healthcare professionals.
• Administer medications and perform procedures such as catheter care, wound care, or hospice support following medical protocols.
• Maintain accurate medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, ensuring compliance with HIPAA regulations.
• Support clients with memory care needs including Alzheimer’s and dementia care by implementing personalized routines and safety measures.
• Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment.
• Use medical terminology accurately when documenting patient information and communicating with healthcare teams.
• Employ patient observation skills to identify potential health concerns early and escalate issues appropriately.
• Work with individuals with developmental disabilities or other special needs to promote independence and quality of life.
• Follow all safety protocols during patient transfers using equipment like Hoyer lifts or other assistive devices.
• Engage in ongoing training related to CPR certification, hospice care practices, medication administration, and other relevant skills to enhance patient safety and care quality.

Experience

• Prior experience working as a CNA or HHA in assisted living facilities, nursing homes, home health agencies, or community-based settings is highly preferred.
• Demonstrated ability to work effectively with seniors and individuals with disabilities across diverse environments such as long-term care or home & community care settings.
• Familiarity with EMR/EHR systems for medical documentation and record keeping is advantageous.
• Knowledge of medical terminology relevant to patient care procedures.
• Experience providing dementia or Alzheimer’s care along with memory support strategies is beneficial.
• Proven ability to handle sensitive situations with compassion while maintaining professionalism under HIPAA guidelines.
• Strong caregiving skills including medication management, vital signs monitoring, catheter care, hospice support, and use of assistive devices like Hoyer lifts.
