Position Summary

The Home Health Aide provides personal care and supportive services to patients under the supervision of a Registered Nurse or licensed therapist in accordance with the physician's Plan of Care and agency policies.

Responsibilities

• Provide personal care including bathing, grooming, dressing, and personal hygiene.
• Assist with toileting and incontinence care.
• Assist patients with walking, transfers, positioning, and mobility.
• Perform range of motion exercises as directed.
• Take and document vital signs when assigned.
• Observe and report changes in the patient's physical or emotional condition to the supervising Registered Nurse.
• Assist with meal preparation, feeding, and hydration.
• Perform light housekeeping related to the patient's care.
• Maintain a clean, safe, and comfortable home environment.
• Document all patient care accurately and on time.
• Follow infection prevention and safety procedures.
• Maintain patient confidentiality in accordance with HIPAA regulations.
• Attend required meetings, training sessions, and continuing education.
• Communicate effectively with patients, families, nurses, therapists, and physicians.
