Home Health Aide (HHA) & Direct Support Professionals (DSP)

Job Summary

We are seeking compassionate and dedicated Home Health Aides (working with the elderly population) & Direct Support Professionals (working with individuals who have developmental disabilities) to provide essential care and support to individuals in their homes. As a Home Health Aide/Direct Support Professional, you will assist with daily activities, personal care, and ensure the well-being of our clients. Responsibilities

- Provide direct patient care under the supervision of a registered nurse/manager

- Assist with personal hygiene tasks such as bathing, grooming, and dressing

- Monitor vital signs and report any changes or concerns to the nurse

- Assist with mobility and transfers, including using lifts or other assistive devices

- Support individuals in their daily activities

- Maintain a clean and safe environment for clients

- Document patient care activities accurately and timely in medical records

- Medication management as needed

- Assisting the individual to & from appointments & grocery stores

Requirements

- Valid certification as a Home Health Aide or completion of an accredited training program or prior experience in the healthcare field.

- Experience working with individuals with DD/IDD and/or elderly population is preferred

- Ability to lift and move patients safely using proper body mechanics

- Compassionate and patient-focused approach to caregiving

- Excellent communication skills, both verbal and written

- Ability to work independently and follow care plans effectively
