Home Health Aide (HHA/PCA)

Duties

• Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support
• Monitor patient health by observing vital signs and recognizing changes in condition; accurately document findings using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
• Administer medications and perform basic medical procedures like catheter care and wound management following medical terminology guidelines and HIPAA regulations
• Support patients with memory care needs, including Alzheimer’s disease and dementia, ensuring a safe environment through specialized techniques in memory care and hospice settings
• Operate medical equipment such as Hoyer lifts for safe patient transfers; assist with mobility using appropriate techniques and safety protocols
• Prepare nutritious meals tailored to dietary restrictions; assist with feeding when necessary while maintaining patient dignity
• Perform light housekeeping duties including cleaning, laundry, and maintaining a safe living environment to promote overall health and comfort
• Maintain detailed medical documentation of patient care activities, vital signs, medication administration, and other relevant records in compliance with healthcare standards
