Responsibilities

• Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary.
• Monitor patient health by taking vital signs, observing changes in condition, and accurately documenting medical information using Electronic Health Record (EHR) systems.
• Support medication administration following medical terminology guidelines and home healthcare regulations to ensure safe delivery of treatments.
• Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care and behavioral management strategies.
• Assist with catheter care, wound care, hospice support, and end-of-life comfort measures while maintaining HIPAA compliance to protect patient privacy.
• Prepare nutritious meals and assist with feeding while adhering to dietary restrictions and safety protocols; perform light housekeeping tasks such as cleaning and laundry to maintain a safe environment.
• Engage in patient observation to detect early signs of health deterioration or behavioral changes; communicate effectively with family members and healthcare teams for coordinated care.
