Responsibilities

• Monitor and report on clients’ health status, observing for any changes or concerns that require medical attention or intervention
• Medication Reminders accurately according to prescribed schedules and document all care activities diligently
• Assist with light housekeeping tasks, meal preparation, and transportation to appointments to promote safety and comfort
• Foster positive relationships with clients and their families by providing emotional support and companionship
• Follow care plans developed in collaboration with healthcare professionals to ensure consistent and effective support
• Maintain detailed records of care provided, incidents, and observations in compliance with privacy regulations

Requirements

• Proven experience in caregiving or home health services, with a strong understanding of patient needs across diverse populations
• Knowledge of Alzheimer's and dementia, emergency response protocols, and basic first aid
• Experience working with clients who have complex health conditions such as geriatrics, post-acute care needs, or chronic illnesses
• Ability to handle emergency situations confidently, demonstrating calmness and quick decision-making skills
