• Monitor clients’ health status by observing changes in physical or mental condition and reporting findings promptly to family members or healthcare professionals.
• Support clients with mobility challenges or disabilities by using proper techniques and assistive devices to ensure safety during transfers and ambulation.
• Maintain accurate documentation of care provided, client progress, and any incidents or concerns in compliance with agency policies.
• Foster a positive environment through active engagement, companionship, and encouragement tailored to each individual’s preferences and needs.
• Collaborate with healthcare teams when necessary to ensure coordinated care plans are followed effectively.
