Caregiver (MUST HAVE PATHWAYS CLIENT)

Job Overview

We are seeking a compassionate and dedicated Caregiver to provide exceptional support to clients within the Pathways program. This vital role involves delivering personalized care that promotes independence, safety, and well-being for individuals with diverse needs, including developmental disabilities, neurodevelopmental disorders, and age-related conditions such as dementia. Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts.
• Monitor patients’ vital signs and observe for any changes in health status, reporting findings accurately while adhering to HIPAA regulations.
• Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary.
• Support clients with medication administration following prescribed care plans and safety protocols.
• Maintain detailed documentation of care activities, patient observations, and progress notes in compliance with healthcare standards.
• Ensure a clean, safe environment by performing light housekeeping tasks such as cleaning client areas and assisting with laundry.
• Provide emotional support and companionship to foster a positive environment that encourages social interaction and mental stimulation.

Qualifications
