Job Summary

We are seeking a compassionate and dedicated Caregiver to provide exceptional support to clients enrolled in the Pathways Waiver program. This vital role involves delivering personalized care that promotes independence, safety, and well-being for individuals with complex needs, including developmental disabilities, neurodevelopmental disorders, and aging-related conditions. Duties

  • Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
  • Monitor patients’ vital signs and observe for any changes in health status, documenting findings accurately in care plans
  • Administer medications following HIPAA guidelines and prescribed instructions, ensuring safety and compliance at all times
  • Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption as needed
  • Support clients with memory care needs, including dementia and Alzheimer’s care, by implementing personalized strategies to ensure safety and comfort
  • Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces
  • Follow established care plans meticulously while providing direct support that respects each client’s dignity and preferences
  • Use manual handling techniques safely during transfers or repositioning, employing tools like Hoyer lifts when appropriate
  • Observe patient behavior for signs of distress or changes in condition; communicate effectively with healthcare teams regarding patient status
