Position Overview

Caden Care Homecare is seeking a dedicated, experienced, and compassionate Lead Certified Nursing Assistant (CNA) to head our client onboarding and care transitions. This critical leadership role is designed for a dependable professional who excels both independently and as part of a collaborative team.

As a Lead CNA, you will be the face of Caden Care for new clients—launching new cases, building vital rapport with families, implementing personalized care plans, and smoothly transitioning permanent caregivers into their roles.

Key Responsibilities

• Case Initiation & Transition: Launch all new start-of-care cases, establish individualized daily routines, and acclimate permanent caregivers to the client's specific needs and preferences.
• Quality Assurance & Oversight: Monitor and evaluate permanent caregiver performance during the first 30 days; conduct field visits and supervisory reviews to ensure compliance with Caden Care standards.
• Mentorship & Training: Assist with staff orientations and provide hands-on, in-the-field training for home care aides.
• Direct Client Care: Assist clients with Activities of Daily Living (ADLs) including light housekeeping, meal preparation, grocery shopping, errands, and meaningful companionship.
• Continuity of Care: Fill in on caregiving cases as needed to ensure uninterrupted service for our clients.
