LPN/Caregiver/Medical Care and PCA Team Coordinator, with private live-in option

Hello! I’m looking for a Caregiver/Medical Care Coordinator / PCA Team Liaison who is part of a small caregiving team in my private home. This position functions as my primary caregiver, medical coordinator, advocate, and team support role. This caregiver will act as a buffer and liaison between me, my PCA team, and healthcare providers, as well as supporting my access to medical care and treatments. You will be an important part of my four to five person caregiving team.

The ideal number of weekly hours is somewhere between 27 and 35, but if needed for the right person we could consider between 20 and 40 hours a week. The baseline version of the job has most of the hours of the job happening between 10 AM and 6 PM, four days a week with Mondays off or possibly another day to be negotiated. If we are looking at more hours or less hours, we would be working off of that framework. Ultimately, the schedule must fit my needs and fit in with the rest of the care team.

All versions of this role include the same core responsibilities, including caregiving, medical coordination, communication with providers, and supporting the overall functioning of my care. The difference between these options is primarily the number of hours, consistency of coverage, and level of day-to-day continuity within the team.
