Responsibilities

  • Design and implement individualized memory care plans tailored to each resident’s needs, preferences, and abilities.
  • Coordinate and facilitate engaging recreational therapy activities that promote cognitive function, social interaction, and emotional well-being.
  • Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.
  • Collaborate closely with nursing staff, social workers, and family members to monitor resident progress and adjust care strategies accordingly.
  • Manage schedules for daily activities, appointments, and family visits to ensure seamless care delivery.
  • Provide counseling and support to families navigating aging issues and dementia-related challenges.
  • Maintain detailed documentation of resident care plans, progress notes, incident reports, and activity participation records.

Skills

  • Proven experience in assisted living or nursing home settings with a focus on memory or dementia care.
  • Strong background in senior care, caregiving, or long-term care environments.
  • Knowledge of Alzheimer’s disease, dementia care practices, and recreational therapy techniques.
