Position Overview

We are seeking a dedicated CNA (Certified Nursing Assistant) to join our team as a Mental Health Professional. This role is ideal for those passionate about working with the geriatric population, supporting behavioral health integration, and providing care coordination for patients in both facility and office settings.

Responsibilities

• Provide care in skilled nursing and assisted living facilities on a daily basis.
• Develop and implement patient care plans using electronic systems, including goal setting and progress monitoring.
• Communicate daily with patients, POAs, facility staff, and families.
• Conduct medication reviews with patients and ask in-depth questions related to their mental health diagnosis.
• Utilize psychiatric scales for assessments.
• Schedule visits for patients requiring additional support.
• Monitor adherence to care plans and evaluate effectiveness.
• Document patient progress in a timely manner.
• Support Behavioral Health Integration (BHI) initiatives.
• Visit patients independently in nursing homes and complete charting in the office setting.
