On-Call Caregiver (DSP/PCA

About the Position

Uplift LLC is seeking a dependable, compassionate, and professional On-Call Float Direct Support Professional (DSP) to provide backup coverage for an individual receiving 24-hour home and community-based services.

This position is ideal for someone who enjoys flexible scheduling and making a meaningful impact. The selected candidate will provide coverage for call-offs, vacations, and emergency staffing needs while helping the individual remain safe, healthy, and independent in their own home.

Responsibilities

• Provide hands-on assistance with personal care, including dressing, grooming, toileting, transfers, repositioning, and mobility.
• Monitor respiratory equipment and follow individualized care instructions.
• Assist with meal preparation, light housekeeping, and maintaining a clean, safe home.
• Support community outings, appointments, shopping, and recreational activities.
• Assist with stretching, mobility exercises, and other daily routines.
• Maintain accurate documentation and communicate changes or concerns promptly.
• Remain awake and attentive during overnight shifts when assigned.
• Follow person-centered support plans while respecting the individual’s dignity, choices, and independence.
• Respond to short-notice staffing requests as needed.

Qualifications

• High school diploma or GED preferred.
• Experience as a DSP, PCA, Caregiver, CNA, Home Health Aide, or similar role preferred.
• Reliable transportation.
• Excellent communication and problem-solving skills.
• Ability to assist with transfers and mobility tasks.
• CPR/First Aid certification preferred (or willingness to obtain).
• Must pass a Minnesota DHS background study.
