Job Summary

We are seeking a compassionate PCW/CNA to provide respectful, person-centered support to residents and clients. You will assist with daily care, observe changes in well-being, and help maintain a safe, clean, and comfortable environment. Your impact: attentive care and clear communication help individuals maintain dignity, comfort, and as much independence as possible.

Duties

• Assist with activities of daily living (ADLs), personal care, mobility, transfers, and manual handling; use a Hoyer lift when appropriate and trained.
• Monitor residents and patients, observe changes, record vital signs, and follow care plans while respecting residents’ rights and privacy under the Health Insurance Portability and Accountability Act (HIPAA).
• Support medication administration within the scope of your role, and respond appropriately using first aid and cardiopulmonary resuscitation (CPR) training.
• Prepare meals, cook, assist with personal transportation, and complete routine cleaning.
• Provide direct support and caregiving in home care, home health, hospice, assisted living, group home, nursing home, memory care, and long-term care settings.

Skills

• Patient care, patient observation and monitoring, client communication, nursing support, and patient transfer methods.
• Experience with senior care, geriatrics, Alzheimer’s and dementia care, memory care, and home and community care.
• Experience supporting individuals with autism, developmental disabilities, intellectual disabilities, neurodevelopmental disorders, or other disabilities.
• Knowledge of meal preparation, heavy lifting, and safe manual handling; ability to follow care plans and protect confidential information.
• First aid and CPR are valued. Relevant experience in caregiving, assisted living, group homes, nursing homes, home health, hospice, or long-term care is preferred.

Pay and Shifts Available

Full time 2nd & 3rd shift position available at an assisted living, Cady Home/Autumn Lane in Birnamwood. Wages start at $14-15 based on qualifications and training. Raise of up to $1.00 available after 90 day probation period based on job performance and attendance. $1.00 per hour for third shift differential. On time bonus of $ 2 per hour just to come to work on time! You could be making $16-$17 an hour if you show up to work on time! $500 sign on bonus after 180 days. We also have a call in pay $5 per hour if you’re not scheduled to work and we call you in. If it’s your weekend off $5 per hour if you work your off weekend!
