Job Overview

Duties

• Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
• Monitor patients’ health by taking vital signs, observing changes in condition, and reporting findings accurately
• Support medication administration following care plans and HIPAA regulations to ensure privacy and safety
• Prepare nutritious meals aligned with dietary needs and assist with meal planning and cooking
• Maintain a clean and safe environment by performing light cleaning duties and adhering to infection control protocols
• Provide companionship and emotional support to reduce feelings of loneliness or isolation
• Document care activities, patient observations, and any incidents clearly in compliance with healthcare standards

Skills

• Proven experience in group home settings, assisted living, or home & community care environments
• Knowledge of patient care techniques including assisting with ADLs, patient monitoring, and medication administration
• Familiarity with dementia care, Alzheimer’s care, memory care, autism support, hospice care, and working with individuals with developmental disabilities or disabilities in general
• Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary
