Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Monitor patient condition through patient observation, vital signs assessment, and reporting changes promptly
• Support clients with memory care needs such as Alzheimer’s and dementia care, ensuring a safe environment
• Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems for accurate documentation of care provided
• Operate assistive devices like Hoyer lifts and perform safe transfers to prevent injury
• Prepare meals, assist with light housekeeping tasks such as cleaning and laundry to maintain a hygienic living space
• Strong patient observation skills with the ability to recognize changes in condition or behavior
• Experience working in assisted living facilities, nursing homes, or home healthcare settings with geriatric patients or individuals with disabilities
• Knowledge of medical terminology, medication administration, and basic life support (CPR & First Aid) procedures
• Familiarity with hospice care, Alzheimer’s/dementia/memory care practices, and working with individuals with developmental disabilities
