Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patient condition through attentive observation and report changes promptly to healthcare professionals
• Perform basic medical tasks such as vital signs measurement, catheter care, and wound management in accordance with medical documentation standards
• Support clients with Alzheimer’s, dementia, memory care, stroke recovery
• Maintain accurate electronic health records (EHR) or medical records using EMR systems while ensuring compliance with HIPAA regulations
• Prepare nutritious meals and assist with light housekeeping tasks such as cleaning and laundry to promote a safe living environment
• Use specialized equipment like Hoyer lifts or other assistive devices to ensure safe patient transfers and mobility support

Skills

• Strong knowledge of medical terminology, medication administration, and home healthcare regulations
• Experience working with geriatric patients, individuals with developmental disabilities or disabilities requiring specialized care such as Alzheimer’s or dementia
• Ability to perform patient observation, vital signs monitoring, and basic life support (CPR/First Aid) confidently
