Job Overview

We are seeking a compassionate and dedicated Personal Care Aide to provide essential support and assistance to individuals in their home health environments. Responsibilities

• Assist residents or clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to ensure comfort and hygiene.
• Monitor patients’ health by observing vital signs, noting changes in condition, and reporting findings to healthcare professionals promptly.
• Support medication administration following care plans and HIPAA regulations to ensure safety and compliance.
• When needed, Help with mobility and transfers using proper patient transfer methods and equipment like Hoyer lifts to promote safety.
• Prepare nutritious meals tailored to individual dietary needs while maintaining cleanliness in the kitchen area.
• Maintain a clean, safe environment by performing light cleaning duties and adhering to infection control protocols.
• Document patient observations, care provided, and any incidents accurately in compliance with healthcare standards.
• Provide emotional support and companionship to residents or clients, fostering a positive environment.
• Assist with transportation or community outings as needed to promote social engagement and independence.
• Support specialized care needs such as bi polar disorder care, memory care, autism support, or neurodevelopmental disorder assistance when required.

Qualifications

• Prior experience working with individuals with developmental disabilities, in group homes, assisted living facilities, or home health settings is highly preferred.
• Knowledge of observation techniques, vital signs measurement, medication administration, and patient monitoring is preferred.
• Experience with specialized care such as behavioral support, crisis care, support with intellectual disabilities is advantageous.
• Ability to perform manual handling tasks including heavy lifting and patient transfers safely using appropriate equipment like Hoyer lifts.
• Strong communication skills for effective client interaction and team collaboration.
• Certification in CPR and First Aid is required; additional certifications such as HIPAA training are a plus.
• Familiarity with HIPAA regulations ensures confidentiality of patient information at all times.
• Ability to work independently or as part of a team in fast-paced environments while maintaining attention to detail.
