Position Summary

As a Hospice CNA, you will work alongside our interdisciplinary hospice team to provide compassionate, patient-centered care to individuals receiving hospice services within a nursing home setting. Your role is essential in promoting comfort, preserving dignity, and supporting both patients and their loved ones through every stage of the hospice journey.

The ideal candidate is caring, dependable, observant, and committed to treating every patient as if they were a member of their own family.

Responsibilities

• Provide exceptional personal care and assistance with Activities of Daily Living (ADLs), including:
• Bathing
• Dressing
• Grooming
• Toileting
• Oral hygiene
• Mobility and transfers
• Assisting patients with repositioning, ambulation, wheelchairs, and other mobility equipment while promoting comfort and safety.
• Observe, monitor, and promptly report changes in patient condition to the hospice nurse.
• Accurately document patient care and observations using Electronic Medical Records (EMR).
• Measure and record vital signs as directed.
• Assist with catheter care, skin care, incontinence care, and other personal care needs within the CNA scope of practice.
