Duties

• Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or manual techniques.
• Monitor patient health by observing vital signs, recording medical documentation accurately in Electronic Health Record (EHR) systems, and reporting any changes to healthcare professionals promptly.
• Provide specialized care for individuals with memory impairments.
• Support patients during end-of-life or hospice care with compassion, dignity, and respect for their wishes.
• Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry, cleaning, meal preparation, and ensuring infection control standards are met.
• Document all patient interactions thoroughly in medical records or electronic systems while adhering to HIPAA privacy regulations.

Qualifications

• Valid certification as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA), with current state licensure if applicable.
• Proven experience working with geriatric patients
• Knowledge of vital signs measurement and Basic CNA skills
