CMT - Memory Care Center

The primary purpose of the Memory Care Center CMT or L1MA is to maintain quality resident care in accordance with the Communities Policy and Procedure and federal, state, and local laws, regulations and guidelines while maintaining the resident’s highest quality of life. Will be responsible for administering medications according to federal, state and local laws, regulations and guidelines as well as assist in providing quality and direct resident care.

This is a full-time, evening shift.

Essential Functions

Department Specific:* Provide quality nursing care to residents in an environment that promote resident rights, dignity, individuality and freedom of choice. * Administer and document all medications by oral, inhalation or tropical route that may be ordered by a doctor. * Conduct resident rounds daily, daily staff huddles and be able to report problems to charge nurse and/or Director of Nursing. * Label all OTC or other medications not marked by the pharmacy that may be used by resident’s * Remove any medications not being used by the resident or add new medications being used that are ordered by doctor per facility policies and procedures. * Maintain confidentiality of all pertinent resident information. * Ensure resident concerns and/or grievances are reported immediately to the charge nurse. * Be able to recognize a change of condition and report on resident’s condition to the charge nurse. * Reorder medications from pharmacist and return all unused medications per facility policies and procedures as well as federal, state, and local laws, regulations and guidelines. * Obtain vital signs as ordered/needed and record appropriately per facilities policies and procedures. * Document all medication errors and report to the charge nurse and/or Director of Nursing. * Assist fellow co-workers in job duties as needed to meet resident’s needs. * Report all incidents/accidents promptly and complete the required paperwork. * Promote positive/open communica...
