This role involves regular travel between Washtenaw, Monroe, Western Wayne, Livingston, and parts of Oakland counties, providing Home-Based Care.

As a Home Care Certified Nurse Aide (CNA) providing in-home care, you'll play a vital role in ensuring our participants receive the highest quality care. From assisting with daily activities to providing comfort and companionship, your work will make a meaningful impact on the lives of those we serve.### Key Responsibilities

• Provide Hands-On Care in Their Home – Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting.
• Maintain Comfort & Care – Support housekeeping tasks such as laundry, sweeping, and grocery shopping.
• Support Mealtime– Help participants enjoy their meals and stay hydrated for good health.
• Keep Things Moving – Support mobility, including turning, positioning, and safe transfers.
• Perform Vital Checks – Measure and record vital signs, weight, and intake/output.
• Maintain Comfort & Care – Keep rooms tidy, cozy, and well-stocked with essentials.
• Provide Responsive Support – Respond with kindness and compassion.
• Monitor with Watchful Eyes – Report any changes in conditions to the leadership ASAP.
• Demonstrate Respect & Professionalism – Maintain confidentiality while delivering top-notch care.
