Your duties may include assisting with daily living activities such as bathing, dressing, and grooming, as well as providing medication reminders and management. You will work closely with our interdisciplinary team of healthcare professionals to develop and implement personalized care plans that meet the unique needs of each patient.

Responsibilities

• Provide personalized care and support to patients in their homes, ensuring their physical, emotional, and social needs are met
• Assist with daily living activities such as bathing, dressing, and grooming
• Administer medications and provide wound care as prescribed by our medical team
• Monitor and report changes in patient condition to our healthcare team
• Maintain accurate records of patient care and interactions
• Develop and maintain positive relationships with patients, families, and caregivers
• Participate in care conferences and team meetings to ensure comprehensive care

Requirements

• High school diploma or equivalent required; additional education or certifications in healthcare preferred
• Ability to lift, transfer, and move patients with care and dignity
• Strong communication and interpersonal skills
• Ability to work independently and as part of a collaborative team
• Reliable transportation and a valid driver's license
