Responsibilities

• Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support
• Monitor and observe patient health status by recording vital signs and reporting changes to healthcare professionals
• Administer medications accurately following prescribed instructions and document medication administration in medical records
• Support patients with specialized care needs including Alzheimer's disease, dementia, hospice/end-of-life care, and memory care
• Operate Electronic Health Record (EHR) systems to maintain precise medical documentation and ensure compliance with home healthcare regulations
• Use assistive devices such as Hoyer lifts and perform catheter care safely and effectively
• Provide companionship and emotional support while assisting with light housekeeping tasks like cleaning, laundry, meal preparation, and ensuring a safe environment

Skills

• Proven experience working with individuals with developmental disabilities, geriatrics, or in assisted living settings
• Strong knowledge of medical terminology, medication administration, vital signs monitoring, and basic life support (BLS) including CPR and first aid certifications
