POSITION SUMMARY

CareForward Home Care is seeking compassionate and dedicated Home Health Aides (HHA) to join our team in Utah. In this role, you will provide direct personal care and support to individuals in their homes, enabling them to maintain independence and dignity while improving their quality of life. You will work under the supervision of nursing staff and follow individualized care plans to assist clients with activities of daily living, mobility, hygiene, and other essential care needs. This position requires a strong commitment to person-centered care, cultural sensitivity, and professional boundaries.

PRIMARY RESPONSIBILITIES

Direct Client Care

• Assist clients with activities of daily living (ADL) including bathing, grooming, dressing, and toileting
• Provide personal hygiene care and assist with maintaining a clean, safe home environment
• Help clients with mobility, transfers, and positioning using proper body mechanics and safety techniques
• Monitor and report changes in client condition, behavior, or health status to nursing supervisor
• Assist with meal preparation and feeding as needed, respecting dietary preferences and restrictions
• Maintain client comfort and well-being at all times

Support for Clients with Disabilities

• Provide compassionate care to clients with physical disabilities, mental health conditions, and acquired brain injuries (ABI)
• Assist clients with cognitive and behavioral support needs in a patient, non-judgmental manner
• Understand and respect the unique needs of clients with traumatic brain injuries, stroke, or other neurological conditions
• Help clients with communication difficulties and encourage independence to the greatest extent possible
• Recognize signs of emotional distress or behavioral changes and report to supervising nurse
• Follow specific behavioral management strategies outlined in individual care plans
