We are seeking a compassionate and dedicated Home Health Aide / Certified Nursing Assistant (CNA) to provide exceptional in-home care to clients across diverse settings. This vital role involves assisting individuals with their daily activities, ensuring their safety, comfort, and well-being. Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Monitor and observe patients’ health status, documenting vital signs and changes in condition accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
• Support medication administration following medical instructions and home healthcare regulations while maintaining strict patient confidentiality in compliance with HIPAA standards
• Provide specialized care such as catheter care, dementia and Alzheimer’s care, end-of-life support, and memory care as needed
• Facilitate safe patient transfers using proper methods and equipment such as Hoyer lifts or patient transfer techniques
• Maintain a clean and safe environment by assisting with light housekeeping tasks including laundry, cleaning, and meal preparation
• Communicate effectively with clients, families, and healthcare teams to ensure comprehensive patient care

Skills

• Proven experience working with geriatric patients, individuals with developmental disabilities, or those requiring hospice or dementia care
• Strong knowledge of medical terminology, vital signs monitoring, medication administration, and basic life support (CPR/First Aid)
• Familiarity with home health agency operations, home & community care practices, and home health regulations
• Ability to operate EMR/EHR systems efficiently for documentation purposes
• Excellent patient observation skills to detect changes in condition promptly
• Physical stamina for heavy lifting and assisting with patient mobility; experience using Hoyer lifts is a plus
• Compassionate communication skills for engaging with clients and their families effectively
