Summary

As a Home Health Aide/CNA, you will provide compassionate in-home care and support to clients, focusing on improving quality of life while maintaining independence. You will report to a supervising nurse or care coordinator and work within established home health regulations to deliver patient-centered care. Your responsibilities include assisting with activities of daily living, monitoring health status, and safeguarding patient confidentiality. With strong skills in patient care, basic life support, and geriatric care, you will help clients stay safe, comfortable, and connected to their communities. You will requiered to drive to patients houses to complete the duties

Responsibilities

• Provide in-home or community-based patient care, assisting with activities of daily living (ADLs) such as bathing, dressing, grooming, and meal preparation.
• Monitor and document patient condition, observe changes, and report to supervising clinician; maintain patient confidentiality per HIPAA.
• Support safety and comfort for geriatric and developmentally diverse individuals; assist with mobility, transfers, and basic life support when needed.
• Maintain clean, organized living spaces; perform light housekeeping, laundry, and meal preparation as required.
• Utilize EMR/EHR systems to update medical records and document care activities; adhere to all home health regulations and care plans.

Qualifications

• CNA or Home Health Aide certification with current CPR/First Aid
• Knowledge of home health care regulations, HIPAA, and patient confidentiality
• Experience in geriatric care, ADLs assistance, and end-of-life or memory care
• Proficient in EMR/EHR systems, medical terminology, and basic medical documentation
• Ability to observe patients, provide compassionate care, and maintain safety in home/community settings
• Strong communication, organizational skills, and reliability in long-term care or home health environments

Pay: $35.00 - $45.00 per hour
