Home Health Aide

Job Summary

As a Home Health Aide, you’ll provide practical, respectful care that helps clients remain safe and comfortable at home. You’ll support daily routines, observe changes in condition, and communicate clearly with clients and care teams. Your impact: By noticing and reporting changes promptly, you help clients and their families make informed care decisions.

Responsibilities

• Assist with activities of daily living (ADLs), personal care, mobility, and safe patient transfers; use a Hoyer lift when trained and appropriate.
• Observe clients, take vital signs as assigned, and document care in electronic health record (EHR) or electronic medical record (EMR) systems.
• Support medication administration only as permitted by applicable regulations and care plans; provide catheter care when assigned and qualified.
• Prepare meals, cook, clean, and handle laundry while respecting each client’s preferences and privacy.
• Provide compassionate support in home health, senior care, memory care, dementia care, hospice, and end-of-life settings.

Skills

• Patient care, caregiving, client communication, medical documentation, medical records, medical terminology, and maintaining confidentiality under the Health Insurance Portability and Accountability Act (HIPAA).
• First aid and cardiopulmonary resuscitation (CPR); basic life support (BLS) is preferred. Follow home healthcare regulations and safe lifting practices.
• Relevant experience may include home care, home health agency, assisted living, nursing home, long-term care, live-in employment, or home and community care.
• Experience with geriatric patients, Alzheimer’s care, or individuals with developmental disabilities or other disabilities is valued.
• Familiarity with patient observation, medication administration, catheter care, and patient transfer methods is preferred; follow assigned duties and care plans.
