Job Description

*TITLE OF IMMEDIATE SUPERVISOR*: DIRECTOR OF CLINICAL SERVICES/CLINICAL SUPERVISOR

*RISK OF EXPOSURE TO BLOODBORNE PATHOGENS* – HIGH

DUTIES

The hospice aide is responsible for providing personal care under the direction of a registered nurse. A person who provides assistance with meals, dressing, movement, bathing or other personal needs or maintenance. Hospice aides must meet the requirements for Home Health Aides in 77 Ill. Adm. Code 245.70 or Nursing Assistants in 77 Ill. Adm. Code 300.660.

RESPONSIBILITIES

Perform personal care (bathing, dressing, toileting, feeding, etc.) according to the Hospice Aide Plan of Care.

Document care provided according to agency policy.

Report any deviations from the Plan of Care to the Case Manager (or designee if not available).

Use Durable Medical Equipment safely.

Take vital signs when on Hospice Aide Plan of Care.
