HOSPICE CNA/LNA - Full time - Chandler, Tempe, Ahwatukee area

Join Our Hospice Team in Phoenix (East Valley)!

We're growing and now hiring a compassionate CNA to visit patients' homes and facilities in the East Valley.

*This is a full time position paid hourly* + mileage.

A Benefits package is available for full-time status (Medical/Dental/Vision, 401K w/ match, basic life insurance.)

Ideal candidates will have experience working with the geriatric population, understand the importance of customer service. We are also looking for candidates who have flexibility for some late afternoon visits for do bedtime tuck-in visits for patients.

Key Responsibilities

• Provide care based on a patient's plan.
• Document on a tablet (company-provided)
• Work closely with RN Case Manager(s) and other IDT members.
• Provide Home Health Aide visits on as needed for our Home Health (non-hospice) patients.

We're looking for a team player with great communication skills. End of Life/Hopsice experience is preferred.
