Responsibilities

* Provide personal care services to hospice patients including bathing, grooming, dressing, and oral hygiene * Assist patients with mobility, transfers, and positioning to ensure comfort and safety * Monitor and report changes in patient condition to the supervising nurse or care team * Deliver compassionate end-of-life care in alignment with each patient's individualized care plan * Support patients and families with emotional comfort and a reassuring presence * Document patient care activities accurately and in a timely manner * Collaborate with interdisciplinary hospice team including nurses, social workers, and chaplains

Requirements

* Current and valid CNA certification in the state of Massachusetts * Prior hospice, palliative care, or home health experience preferred * Strong understanding of end-of-life care principles and patient dignity * Excellent communication and interpersonal skills with patients and families * Reliable transportation and ability to travel to patient homes/long term care facilities in the Waltham, MA area * Ability to work independently while remaining a collaborative team member * Compassionate, patient, and emotionally resilient demeanor
