Job Title: Hospice - Full-Time Certified Nursing Aide (CNA)

Location: San Angelo, TX, 76901 Agency: High Plains Senior Care Hospice Job Summary: Primary function is to provide healthcare and related services to the patient in their place of residence; assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor. Responsibilities: * Promote hospice mission, goals and philosophy. * Demonstrate commitment, professional growth and competency. * Demonstrate compliance with policies, procedures, regulations defining scope of practice. * Promote security and safety of Protected Health Information. * Participate in programs and on committees as requested. * Participate in ongoing education as required. * Provide services according to the Hospice Aide Care Plan. * Coordinate with RN-develop/update plan of care to meet the needs. * Attend IDG meetings, as requested. * Document services and submit timely. * Other duties as assigned. * Report changes to the RN.

Qualifications

* State Driver’s License . * At least one year preferred, as a hospice aide or nursing assistant in a hospital, nursing home, home health agency, hospice, or long term care facility. * Strong communication and interpersonal skills; Sensitivity to issues of loss and grief; Ability to deal effectively with the demands of the job and population serviced; Ability to work without consistent direct supervision. * Reliable transportation. * CPR certified; TB health screen or TB test/CXR according to policy; Hepatitis B Vaccination Consent/Declination
