In Home Caregiver

Overview

We are seeking compassionate and dedicated In Home Caregivers to provide personalized care and support to clients in their own homes. The ideal candidate will have experience working in diverse healthcare settings, including and post-acute care environments. This role offers an opportunity to make a meaningful difference in patients' lives by delivering high-quality, patient-centered care across various medical disciplines such as geriatrics, medical-surgical, outpatient clinics and sub-acute care. This position is paid and suitable for individuals committed to providing exceptional home health services.

Responsibilities

• Provide comprehensive in-home care tailored to each patient's individual needs, including assistance with activities of daily living and medication reminders, light house keeping, companionship
• Monitor patient health status and report changes promptly to healthcare providers
• Assist with mobility, hygiene, nutrition, and comfort measures to promote well-being
• Maintain detailed documentation of care provided and patient progress
• Collaborate with healthcare teams to ensure continuity of care across inpatient, outpatient, or post-acute settings
• Adhere to all safety protocols

Requirements

• Background working in hospital medicine, emergency medicine, inpatient or outpatient clinics and homecare settings is highly desirable
• Strong communication skills and compassionate bedside manner
• Ability to work independently while following established care plans
• Valid driver’s license and reliable transportation for home visits
• Ability to pass background checks and meet all health screening requirements
• Certification as a Certified Nursing Assistant (CNA), or some experience in homecare.

Pay: $16.00 - $20.00 per hour
