Responsibilities

• Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
• Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation within Electronic Health Record (EHR) systems
• Support medication administration following proper protocols and respecting patient confidentiality in compliance with HIPAA regulations
• Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care and behavior management strategies
• Assist with medical procedures such as catheter care, wound dressing, and hospice/end-of-life support while ensuring patient comfort and safety
• Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry and meal preparation in accordance with home & community care standards
• Document patient progress meticulously and communicate effectively with nursing staff regarding any concerns or changes in condition
