Duties

• Assist residents and clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
• Monitor patient health by taking vital signs including blood pressure, pulse, temperature, and respiration; report any irregularities to healthcare professionals
• Support medication administration following proper protocols to ensure safety and compliance with HIPAA regulations
• Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking to promote wellness
• Maintain accurate care plans and document patient observations diligently to support ongoing care strategies
• Provide companionship and emotional support to reduce feelings of loneliness or confusion, especially for individuals with dementia or Alzheimer’s disease
• Perform light cleaning duties within the resident’s environment to uphold hygiene standards and promote a safe living space

Skills

• Proven experience in home & community care settings or long-term care environments such as nursing homes or assisted living facilities
• Knowledge of patient observation techniques and ability to recognize changes in condition or behavior
